Повідомляємо, що реєстрація вже відкрита для УЧАСНИКІВ Лещетарсткого табору 2022!

 

Зголошення складаєтся з двох частин:

 

1- Електронна частина (ця форма) і "e-mail of scanned pages"

        частина Є ОБОВ'ЯЗКОВА, щоб завершити аплікацію на табір.

2- "e-mail scanned pages"– включає:

             • Виповнена медична форма і копія медичної карти 

                (insurance card front and back)

             • Копія COVID-19 вакцинації

             • Підписану посвідку пластового виховника і підписану

                посвідку “Release of Liability”.

             • КПС - "Code of Conduct"  і "Waver and Liability Release"

 

THIS APPLICATION HAS 2 PARTS:

1- The e-form (this form). This e-form is only the first part of two. The

         second part consists of scanned and e-mailed pages. The e-form

         alone will not be sufficient. You must submit BOTH parts for your

         application to be complete.

2- The e-mail scans should include:

             • Filled out medical form and copy of insurance card

                (front and back)

             • Signed recommendation form from Plast counselor and

                “Release of Liability”.

             • KPS - "Code of Conduct"  and "Waver and Liability Release"
